The Cycle of Engagement Model and Tools were designed and tested by the Child and Adolescent Health Measurement Initiative in partnership with families, providers, experts, and researchers.

The Child and Adolescent Health Measurement Initiative (CAHMI)

The Child and Adolescent Health Measurement Initiative (CAHMI) is a national initiative started in 1996 to keep the focus on children, youth, and families in health care and public health services, systems, and policy change. We focus on engaging families, providers, and system leaders to build the supply of health outcomes and systems performance measures, advance new knowledge into policy, and develop and test translational tools to improve systems and care. We work to disseminate and build the demand to use family-centered, outcomes-oriented data and tools to inspire, inform, and support transformational partnerships that aim to improve the early and lifelong flourishing of children, youth, families, and communities.

Our Mission

The mission of the CAHMI is to promote the early and lifelong health of children, youth, and families through the translation of the science of healthy child development and thriving into public health and health services research, policy, and practice.

How We Do Our Work

The work of the CAHMI is guided by our values: (1) equity and positive health development for all children, youth, families, and communities; (2) child, youth, and family partnership and engagement at all levels; (3) translation of science, data, and evidence into policy and practice; (4) cross-sector collaboration and innovation to achieve culture and systems change

Cycle of Engagement Well Visit Planner Approach to Care (COE WVP): Our Journey and Commitment

The COE WVP has been a labor of love for the CAHMI. We are driven by our dedication to partner with you to meet the great need and possibilities to promote the early and lifelong health of children, families, and communities. As a part of our broader Advancing Tools and Technologies to Achieve Child Health (ATTACH) work, since 1997 the Child and Adolescent Health Measurement Initiative (CAHMI) has partnered with families, providers, and experts at the national, state, health plan, practice, and provider levels to design, develop, and test the COE model and tools. With initial support from The Commonwealth Fund and the Health Resources and Services Administration, CAHMI has worked in close collaboration with Family Voices and leaders of the American Academy of Pediatrics’ Bright Futures Guidelines for Health Supervision of Infants, Children and Adolescents to translate guidelines into family-friendly, actionable tools to customize and improve the quality of well child care services based on family reported assessments and priorities. The Well Visit Planner includes standardized assessments, including the Survey of Well-Being Young Children (SWYC), for which the CAHMI holds a license for use in the COE WVP. Other guideline-aligned content was designed and tested by CAHMI.

Be A Partner: Cycle of Engagement Well Visit Planner Approach to Care
